Diabetes & Cardiovascular Evaluation

Abstract:
-------------

Diabetes mellitus is still increasing worldwide and the IDF estimation of 2011 shows 366 000 000 patients.

The cardiovascular risk and mortality of diabetes patients is still minimally double compared with no diabetes.

Risk indicators are extremely important to show which patients need most aggressive treatment.

Important risk prameters are of course blood pressure, cholesterol, smoking, age, but more recently sCRP, troponin-T are showing some evidence. Even so microalbuminuria and erectile dysfunction are proven strong risk indicators risks for myocardial infarction and stroke and recurrence.

It is important if we can correlate this markers with increased risk and if changing markers, especially with diet, medication, life style, surgery…will have an influence on the prognosis. Also measurement of advanced glycated endproducts will modify our UKPDS-risk scale(and treatment).

Clostridium Difficile - Challenges in Management & Prevention

Abstract:
-------------
•Clostriduim difficile is a gram positive bacillus which forms spores and produces toxin A and B,which are responsible for illness.
•It causes mainly diarrhea and psedomembranous colitis especially in patients who had been given prior antibiotics.
•The prevalence is high in critical care hospitals and long term care areas, where the use of empiric antimicrobial therapy is very common
•The diagnosis is easy with toxin assays, stool culture, detection of antigen and PCR techniques
•Early diagnosis is the key to successful management as complications can be severe.
•Treatment includes discontinuation of provolking antibiotics. Also oral vancomycin and metronidazole are indicated.
•Most important is prevention as it is highly communicable. Strict contact isolation precautions can play a major role.
